PHS Islands Association 2012 Annual Survey

The Board of Directors is conducting our Annual Survey of residents in order to determine where to dedicate the Association’s time and resources in the coming year.  Your cooperation is requested by taking a few minutes to fill out this form and indicate those areas which are most important to you.  The Board places great  emphasis on the results of the survey.  In this way, we can ensure that we incorporate your desires into our work for 2012.
